Form G-1145 is a U.S. Citizenship and Immigration Services (USCIS) form also known as E-Notification of Application/Petition Acceptance. USCIS offers e-notifications for applications filed at USCIS lockbox facilities. You can download Form G-1145 on USCIS.gov. With this form, you can opt for electronic notifications (via email and/or text message) from USCIS.
Receiving e-notifications from USCIS is a great way to keep up with your case status information. You’ll get an application receipt notice and you’ll know when USCIS accepts your immigration forms for processing.

You should only file Form G-1145 with applications submitted to a USCIS lockbox facility. All family and marriage green card applications fall under this category. The form is optional, but if you don’t opt for e-notifications, you’ll have to wait an extra week for USCIS to notify you via mail.
The form is very brief. You’ll only need to provide your full first name, middle name, last name, email address, and mobile phone number. You must place Form G-1145 on top of the first page of your immigration application package when you submit it. USCIS will then send you an e-notification within 24 hours of accepting your application for processing.
There is no fee to file Form G-1145. You’ll only need to pay the correct fees for the immigration benefit to which you’re applying. You can pay any fees using either a check or money order or a credit card or debit card.
